LIfting the Jargon

The discussion of rejuvenating the facial contours to produce a softer, younger appearance can begin and end linguistically. Words and labels are powerful, as they direct our decisions and emotions. Patients have expectations of which words should be on the menu, i.e. Botox, Restylane, chemical peels, microdermabrasion, laser resurfacing, photorejuvenation, skin tightening, but oft times may not completely understand the premise and function of the procedure they are purchasing.

This issue is amplified in the jargon of facial lifting as the "liquid lift", or "non-surgical face lift", or "Dr. X lift", or "laser lift", or "weekend lift", or "mini lift", or "Botox brow lift", or ad infinitum lift, create ambiguous impressions and ask for demystification. Sophisticated consumers desire details of what will be done and what it can do. "Lift" labels beg these questions; so let's lift labels.

To address the most common label, non-surgical face-lift, background is necessary regarding surgical lifting. Surgical face lifting is in its typical application, surgical redraping of skin and underlying tissue over sub-structures of muscle and bone.

It works beautifully, when properly applied, to erase folds in the skin, decrease shadows, and redefine facial architecture. Limitations are inherent, in that decreasing the dress size does not give back the curves of youth. Implants have been designed to address this limitation. In my opinion, the term "lift" could be left in this surgical arena, thereby giving these procedures back their namesake, decreasing confusion, and allowing the arena of volume restoration its own definitional heritage.

The non-surgical face lift might be more aptly termed non-surgical facial rejuvenation, which when categorized generally includes three types of procedures, the first and most effective being volume replacement under the skin surface with dermal fillers, such as Restylane, Radiesse and a number of others. These products "fill" with volume to create more youthful contours, and can be dosed to the desired aesthetic effect very readily, allowing both physician and patient tremendous latitude in the range of modification.

The second general area of the "non-invasive" rejuvenation could be relegated to mention of Botox (Botox Cosmetic, Allergan), the most popular cosmetic procedure on the globe. It can be effectively used to relax wrinkle-causing muscles, is extremely safe, effective and when used in concert with dermal fillers (such as Restylane or Radiesse) provides very elegant results.

The third and most confusing area is that of energy-based "skin tightening", the premise being stimulation of the lower part of the skin, the dermis, to tighten itself, i.e. energy induced "lifting". These procedures work with variable efficiency to tighten already existing collagen and to stimulate fibroblast cells in the dermis to secrete more collagen. Examples of procedures performed to this end include Thermage, Fraxel, Pixel, Portrait, and others. Modest tightening is the general rule with these procedures and it has been difficult to predict which patients will respond best.

I hope then that this short introduction serves to help peel back lift labels, and strengthens the informational bridge between aesthetic providers and our intelligent and valued clients. We as educators - physicians, nurses, aestheticians, and laser technicians, alike - should all always strive to provide our patients/clients with explanations of the "mechanisms" of our "lifts", thereby improving understanding, expectation, and satisfaction with these culturally important and increasingly sought after services.
